Cave Story. Though for whatever reason OrgView screws up the music a little (and yet the actual game doesn't!).
Funny: IIRC, when I actually first played this game, the idea of a 3 HP run (or a 3 HP/minimal items run - getting the good ending, of course) was purely hypothetical... now there's guides on how to do it.
